PLEASE READ AND FOLLOW ALL INSTRUCTIONS CAREFULLY.  PLEASE DO NOT SEND REGISTRATION FORM ELECTRONICALLY OR BY POST. THIS FORM MUST BE HANDED IN PERSONALLY TO MAIN RECEPTION.

If you are resident in the practice area, you must complete a registration form to join the surgery, and any preference you may have of practitioner will be noted. When you register with us we will need to see photographic evidence of your identification, and also confirmation of your address.

Online 'Pre-Registration' With The Practice

If you wish to pre-register click on the links below to open the forms. When you have completed all of the details on each form, click on the "Send" button to mail your form to us. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct.  We also need you to bring along 2 forms of identification before we can register you.  One needs to be photo id and the other confirming your address.  PLEASE NOTE - WE ARE UNABLE TO REGISTER YOU UNTIL WE HAVE ALL THE NECESSARY FORMS COMPLETED.

STEP 3 - Visit reception at Oak Tree Surgery with your identification documents as detailed above so that your registration can be completed.

Note that by sending the form you will be transmitting information about yourself across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.
